Aron starts to become delirious as the hours tick by - first hearing things, then seeing things (including a blow-up Scooby Doo). He watches footage from his video camera to keep him grounded, but starts to have premonitions about meeting his future wife, and having a son together.
He grits his teeth and straps a tourniquet around his arm. With his pocket knife, which has been ground down to a stub, he cuts open his right arm, and then starts to saw and tug away at the bone and tendons.
We see the pain and determination he goes through, regularly taking breaks as he almost passes out, until finally he can step back. He ties up his right arm in a plastic bag and covers it with a part of his backpack.
He takes a picture of the gory scene as a memento, then fumbles his way out of the ravine.
Soon Aron spied a pool of water down a cavern. Luckily, climbers have already been there, so he uses their hooks and expertly abseils down with one arm, and then dives head first into the dirty water. He stocks up and then goes walking.
Almost at breaking point, he summons all his strength to shout out to a family rambling by in the distance. They run to help him. Soon they meet up with more hikers (must be like buses), who run ahead to get help, until a chopper arrives to airlift him out of there.
Later, we watch him swimming in a pool to meet all of his family in some type of dream sequence.
Before the end credits we see the real Aron Ralston, and we're told that 3 years later he met his wife, and they had a child. He still goes rock climbing -- but this time has learnt his lesson and always leaves a note to tell others where he'll be!
